I am the founder of June Care, a digital care platform that empowers families to support their relatives diagnosed with Alzheimer's or dementia. We've built a digital therapeutics program made to manage and change behavior around caregiving. Dementia affects 1 in 10 seniors and is the 6th leading cause of death. We help health systems identify the population at risk for preventable readmission and enable families to drive better health outcomes for their affected relatives. Each family caregiver is placed in a cohort alongside other participants. Geriatric clinicians lead behavioral therapies and offer continuous coaching. We eliminate the barriers to evidence-based, cost-effective intervention and bridge gaps in the system for longer aging in place.
